Extract Resource Token

This extracts the Resource::Token from Resource and applies some various
cleanups:

- There is now Resource::AllOfType rather than a WildcardConversion
- Remove Resource::Conversion entirely
- Resource::ALL now holds the magic "(all)" string
- We now test that the types and IDs are coerced to strings
- Add a NoIdentifierError for Agents and Resources that do not have a
  known way to get an ID
- Add eql? and == methods for Agent and Resource
- Some various test cleanups for clarity
- Lots more YARD comments
