[FIXED] Server in cluster mode may Panic due to raft timeout

- We were using a timeout to enqueue raft.Apply() commands, while
  we should really not (this could leave to an inconsistent state
  between the raft and streaming logs).
- The replication functions were not really reporting to the clients
  errors due to the processing of the command, only what the low
  level raft failures.
