Feature/hook concurrency (#7)

* Add command concurrency control, refactor Hook to be part of server package instead of event package, improve logging and commandResult fields

* Add example with sleep and false command, add server/workers file

* Refactor server to encapsulate http.Server

* Make sync request respect concurrency parameter

* Remove go1.7 from tests as it does not support server.Shutdown
