Prevent multiple precache manifest in webpack development (#1276)

* Prevent `importScripts` from including more than one precache-manifest
path in development. As Webpack dev server may emit more than once if
changes are made to the compilation source.

* Adopt a cleaner syntax for pushing an array to an array; Adding test to
cover the Webpack recompile use case

* Fix the same issue with generateSW, where a Webpack re-compilation in
development will cause the `importScripts` to have multiple duplicated
copies of precache-manifest as well as workbox-sw path
