Fix bug whereby excluded modules were not correctly honoured. (#138)

* (M) ygen/codegen.go
 * (M) ygen/codegen_test.go
   - Fix bug whereby for root-level elements, exclude modules arguments
     would not be honoured resulting in struct fields that did not exist
     in the schema being present.
 * (M) testdata/structs/excluded-module.yang
 * (M) testdata/structs/excluded-module-two.yang
 * (M) testdata/structs/excluded-module.formatted-txt
   - Add test data for the above bug.
