improve loading of app.psgi in MetaCPAN::Web::Test

Rather than adding the current directory to @INC, use a relative path
(./) to load app.psgi.  Also switch to do rather than require to make it
clear we care about the return value.  Also cache the resulting app in
case someone calls the method multiple times.
