Rework of shutdown paths

Client and Connection now both use a split close/wait_closed approach,
similar to asyncio.Server. Connection also eliminates the start method
and hides its internal task, with wait_closed doing the work instead.
Connection is also simplified by never having its task set to None, even
when closed.

Close functions now take care to only cancel the task once, which avoids
the risk of cancelling a cleanup handler handling the first
cancellation.

Server should also get similar treatment, although it will break
backwards compatibility.
