Add support for refreshing the remember me cookie on every request. (#359)

Similar to Flask's `SESSION_REFRESH_EACH_REQUEST`, but for the remember
me cookie.

Off by default, can be turned on by setting the config parameter.
`REMEMBER_COOKIE_REFRESH_EACH_REQUEST` to `True`.
