Remove mention to migration guide and changelog from README

As the v1 release has been out for a while and no one complained about migrating
pre-v1 versions, writing a guide might probably is not useful. We might be
wrong, but I'm sure our users will let us know.

I also removed the Changelog as it doesn't add any useful information now. As we
release more versions, we can bring it back.
