Build process:
* Install satooshi/php-coveralls before builds on travis-ci

Composer:
* Removed dev dependencies:
   - satooshi/php-coveralls
* Added dev depencies required for some tests:
   - doctrine/orm:^2.5
   - doctrine/doctrine-bundle:^1.6
   - doctrine/doctrine-cache-bundle:^1.3
   - symfony/swiftmailer-bundle:^2.0.13
   - sensio/distribution-bundle:^5.0

Bundle:
* Added more documentation
* Added more bundle parameters
* Added Generator and Manipulator classes for use in console commands
and test suite
* Added a global router:match command
* Added a replacement for the generate:bundle command to place tests in
the correct
  directory
* Refactored how the BootKernel reads and generates its kernels.php
cache file
* Refactored request handling
* Moved abstract test base classes to tests namespace
* Removed class AddKernelsToCachePass since a change in the kernel list
does not
  require a container rebuild anymore

Tests:
* Added testdox annotations
* Added a PHPUnit Listener to:
  - generate a test environment under /tmp from templates before tests
are run
  - remove the test environment after all tests are run
  - pre-build container classes before each test when required, to speed
up the
    test suite when generating a coverage report
* Removed fixtures directory
* Removed tests for the abstract test base classes
