Change the interface for metadata and attribute indexes

The original design of the API had the metadata/attribute index
access as iterators through series of dictionaries. This has been
changed to just directly return the nested dictionary of the
indices: this simplifies the code and the usage.

The original iterator approach is unnecessary: the worst case
granularity for our current dataset sizes is n_frames x number
of columns, which is unlikely to be a significant memory burden.

All of our downstream uses of this function materialise the whole
index anyway.
