Upgrade to Restify 6.x

- Replace custom CORS implementation with CORS middleware
- Remove `mapParams` property from `bodyParser` because:
> By default, queryParser and bodyParser no longer map req.query and req.body to req.params.
- Change request logger, body parser, and query parser to be under `.plugins` namespace
