Fix namespaces (#147)

* Fix createKeySection to generate the ds prefix properly. Update test case as well.

* Make sure EncryptedAssertion has the proper namespace URI in encryptAssertion

* Add xmlns:xsi and xmlns:xs namespace definitions to <Assertion/> in defaultLoginResponseTemplate, useful for <AttributeValue/> inside <Attribute/>
