Inject Cornerstone as a dependency (#271)

* Instead of injecting we can just change how externalModules.js works, since it is already a center point to get the cornerstone. This would be less traumatic

* Changing implementation to use getter and setter for conrnerstone

* Removing useless files

* OHIF-128: fixing examples with new metaDataProvider

* OHIF-128: using cornerstone directly, it is the same instance

* OHIF-128: got externalModulesES6modules.js again

* OHIF-128: using window.cornerstone as default value
