Reimplement fix for #102 - always check server state at StateUpdateFrequency.

New opst.stateUpdate method is now the only thing that calls server.Alive(true),
and does it in a goroutine, since that can be slow.

provider.Spawn reverted to not take a check frequency. cloud.Server.RunCmd
reverted to not check if Alive().

cloud.Server has new GoneBad(), NotBad() and IsBad() methods. Alive() no longer
calls Destroy() if it doesn't seem alive.

scheduler has new SetBadServerCallBack() (implemented for OpenStack scheduler)
and new SetMessageCallBack() (not yet used).

standin.waitForServer now returns an error, and failure reasons are now kept
and reported through with new constant standinNotNeeded.
