Switch to CRNA, CRA & React 16 (#26)

Fixes #9 & closes #25 

**HUGE** code re-architecture:

- Use Create React Native App for building React Native
- Use Create React App for building React on the web
- Upgraded to React 16 (Fiber)
- Moved to a single screen with the route picker at the top
- Shrink font size so it fits on iPhone 5

This sets the foundation to redo the entire UI based on a more material UI look and feel (i.e. copy Google Maps).
