Feature/always use generateLutNew algorithm (#169)

* generateLut always uses the code of generateLutNew, instead of only when modalityLUT or voiLUT is undefined.

It simplifies the code by removing duplicate, and it does not change the behavior because generateLutNew does the same calculation when modalityLUT and voiLUT are undefined.

* remove generateLutNew.js

* Resolve the error "Error during loading "karma-phantomjs-launcher" plugin: Path must be a string. Received null", throwed when submiting the PR

* generateLutNew_test calls generateLut
