Add libphonenumber format/intlFormat field support

This stores the format and intlFormat given for each libphonenumber
formatter in the stub country files, so they can be used when format()
is called rather than only joining the parts with space.

This also changes the NANP format output to match.

Pull Request Pull Request #80: Add libphonenumber 'format' field support.
