[NEWRELIC] initial new relic support

briefly: when calling middleware.dispatch, the optional 4th & 5th
parameters refer to a newrelic singular-instance and a string prefix
(e.g., “emissary”, “heartbeat”, etc.)

if the newrelic parameter is non-null, then the appropriate newrelic
calls are made to record HTTP requests and request-related errors
