fix an odd Content-Length  & Content-Type bug

 there's a weird bug where these might exist in $_SERVER regardless of wether the client sent these headers or not.
 when they were not sent, they contain emptystring. when they do contain emptystring,
 i prefer to assume that they weren't sent, rather than they were actually sent with emptystring
 (i think no SANE http client sends them with emptystring anyway. a SANE client will send content-length: 0 - not content-length: emptystring)
 ( and afaik its impossible at this point to figure out which is the case)

- observed on Debian 9 with nginx 1.10.3 & php-fpm 7.0.19
