Refactor lazy initialization of 'this._userIgnored'

- Avoid crash when 'options.cwd' exists and 'options.ignored' is not an array.
- Reduce creation of new arrays to 2 maximum (used to be 6 max).
- Add a comment clarifying the possible types of an ignored criterion.
