setup: Upgrade to npm v5.0.0

npm v5.0.0 now automatically saves installed packages to package.json
and creates a package-lock.json file. Consequently, I've removed the
complexity from the `./go setup` script and related scripts, opting to
always install Karma and its related packages even if the user won't use
them. If the additional overhead ever becomes a concern, I can consider
how to manage it at that time.
