Add pulp-smash cli

Do at least the following:

* Drop calling pulp_smash module using `python -m pulp_smash`. In other
  words, remove the `pulp_smash.__main__` module.
* Add click as a requirement
* Add `pulp_smash.pulp_smach_cli` module. This is the place where the
  Pulp Smash's CLI code is located.
* Update documentation to reflect the previous changes.
* Add tests for the new `pulp_smash.pulp_smash_cli` module.
