* refactor(Trade): separate setFromAPI() from constructor

* refactor(Exchange): take on more responsibility

I had to move some of the constructors around.
Also created a reusable Trade mock.

* build(Index): don't expose mocks

* fix(Exchange): getTrades() requires QuoteClass

* refactor(Trade): sell() uses setFromAPI

* test(Trade): API reference

* fix(Trade): sell() should use res
