Clean up query.ex and improve coverage. (#171)

* Update moduledoc to match Postgrex.

* Eliminate concept of decoders.

No longer present in Postgrex version of this file either.

* Match PG behavior for parse and describe.

* Remove concept of decoders. No longer exists in PG.

* Remove special case for :blob data type.

It was not meaningfully different from the fall-through case below.

* Add test coverage for time-valued fields.

Looks like it's not possible for adapter to see the precision of subsecond time values, so we'll remove code that handles that case.

* Remove unreachable case for translate_value/1.

* Add case for decimal without specified precision.

* Code coverage is 97.8%. I'm gonna call that "improved."
