Fixes #162 - PMD rules in Maven don't match Eclipse

The Eclipse plugin doesn't like the rule reference syntax.
Copied the rule definitions from the various individual
files into the main pmd.xml. That allows both Maven and
Eclipse to builds to work off of the exact same definition.
