feat: remove insert method

It is of little value when Postgres already provides returning. In such case, using `.one` makes a lot more sense, e.g.

```js
const {id} = await connection.one('INSERT INTO person (name, tmdb_id, imdb_id) VALUES (?, ?, ?) RETURNING id', [
    person.name,
    person.id,
    imdbIdTokens.id
  ]);

```
