Created meta-command "expressive"

Allows usage such as the following:

```bash
$ ./vendor/bin/expressive module create Foo
$ ./vendor/bin/expressive create-middleware "Foo\BarMiddleware"
```

The command includes help, which lists all commands, and a way to use
the standard syntax `expressive help <command>` to proxy to the help for
individual commands.
