Initial logging integration (#824 & #794)

Integrate basic logging as middleware. This requires modifying our
middleware chaining logic to flatten chains as necessary.

At some point, we may want to move logging and metrics from middleware
to first-class transport and encoding concerns. That approach is more
laborious and doesn't work well with third-party implementations, but it
has the virtue of being more accurate. Right now, though, it's more
important to get consistent coverage.
