* Initial logging integration

Add some logging on dispatcher startup; the next PR will introduce logging in
the request path.

* Update dependencies

* Add a simple logging middleware

* Unexport logging config

* Update dependencies again

* Make observing middleware unaware of other middleware

* Use chains in dispatcher

* Fixes from @willhug

* Update glide again
