Ignore null (#8)

Update the JSON generator so that it can ignore null fields.

Summary of commits:
    * Fix a crash in JSONGen when keys field ends up empty

    * Provide jsonGen utility

        A simple binary which can produce outline parser code by wrapping the
        SimpleParsedJSON_Generator class

    * Initial support for ignoring null

        Initial support for ignoring null. This inital fix is missing
        tests, which would break (null at start of arrays, empty arrays)

    * Better support for ignoring nulls

        - Ignore empty arrays
        - Type arrays if null the first item

    * Additional Tests for null objects

        Add an additional test to ensure we can handle nulls in an array of
        objects
