Get rid of mmt-gensym (#3)

* Add .cask to .gitignore

* Use cl-gensym internally instead of mmt-gensym

This will allow users who only use mmt macros to not require mmt at
runtime after compile.

* Skip loading cl-lib at runtime

mmt does not need cl-lib at runtime after compile, so skip it.

* Remove mmt-gensym

This is identical to cl-gensym in cl-lib, which ships with Emacs.
There’s no need to provide duplicate names for this within the Emacs
Lisp community.
