* Fix #2659: File attachments shown twice after migration

Note: After fixing the double migration, it turned out that the
sequence wasn't being set either.

Note: While 'file_base' should not have any records of its own,
  nothing has prevented this in the past; which is why we're
  choosing to migrate all records from file_base which are not
  in any of the inherited tables. (Instead of not migrating at all.)
