Fix several issues (#13)

* Revert Justin’s changes to migration files

* Rename spec factories

* Fix indentation and add custom_canvas_account_id to settings

* Add tenant field to application_instances

* Add tenant to seeds.
Fix url to pull from secrets.

* Fix Apartment middleware to return the tenant instead of lti_key

* Re-structure json in seeds so adding more data is easier

* There, is that better? (MIB)

* add default random tenant when creating application_instance

* Use secrets for app_url instead of env as secrets pull from .env for that value.
