Minimal out-of-court skeleton app

* Organise app structure and add start/exit page
* Add minimum Javascript to get radio buttons to work
  * JavaScript taken from https://github.com/alphagov/govuk_frontend_toolkit
  * Asset build step will come later, if needed
* Google Analytics middleware into app_spec
  * doesn't distinguish between staging and production anymore
* GitHub repo name changed - change references to repo
  * Fix coveralls
  * Configure travis to run merged coveralls
