Fix Android tag data loss on database upgrade and missing tag loading (#607)

Replace fallbackToDestructiveMigration() with an explicit Migration(10, 11)
that creates the five new planner tables, preserving all existing user data
(including food tags) when upgrading from schema v10 to v11.

Also fix FoodLogEntity.toModel() and RoomFoodLogRepository to load tags
from the food_log_tags join table so FoodLog.tags is populated correctly.
