fix: name the UTCTiming scheme of every clock source

The report said "Clock: synced to https://time.akamai.com/?iso&ms" without
the UTCTiming scheme, and said the same when no UTCTiming of the MPD worked
and dashcheck followed the reference clock.

Measurement.Source now names every source by its scheme, such as
"UTCTiming http-xsdate https://time.akamai.com/?iso&ms" and "UTCTiming
direct (the time in the MPD)", and CLOCK-SKEW uses that name. Without a
working UTCTiming the source is "the reference clock <URL>".
