Merge branch 'fix-eap-auth'

This fixes server-side configuration of EAP authentication and adds
the ability to request/match EAP-Identities.  It also adds migrations
to the repository so we can track database changes in the future.
Finally, it uses the default proposals for new connections.  We could
potentially make these configurable in the future.
