Closes #3: Not Working on Windows

- Configuration for Appveyor CI running on Windows
- User 'sh' to spawn shell commands (we expect Git and Git-bash to be installed
- Change testcases to call node mockups using `node <path-to-file` as command
  since shebang lines don't work directly in Windows as they do in Linux
- Adapt more testcases, replacing '\' to '/' in the contents of the `GIT_EDITOR`
  environment variable
- Remove platform distinction in q-child-process (not needed when using Git-bash)
- Send Appveyor build data to coveralls
- Include Appveyor-badge in README.md
