docs: update README to document Port Monitor feature and UI changes

- Add Port Monitor pane description to features section
- Add -p/--port flag to options table
- Add usage examples for port check (single, comma-separated, with -T)
- Document Port Monitor pane columns (Port, Service, Status, Open/Closed, Last Change)
- Update Tab key description to include Port Monitor pane
- Remove Hostname column from display items (now merged into Dst IP)
- Rename Traceroute pane to Traceroute Monitor pane
