docs: rewrite DEPLOY.md for single-VM deployment

Consolidate the two-VM (Conduit + Vault) layout into a single-VM
setup. Merge the split VM① / VM② sections into one linear flow,
unify <VMのIP> references, and document that VAULT_ADDR must use the
host LAN IP since the backend runs inside a container.
