fix: decouple theme from init()

Replace the init()-based role resolution (which panicked via
index-out-of-range on missing palette colors) with injected NewTheme(name,
palette, roles) returning an error. Add NewMonokaiTheme and keep MonokaiTheme
as a wrapper for backward compatibility.
