Fix shell patterns in pl.dir

* Anchor lua patterns produced from shell patterns at the beginning.
  "abc" now does not match "b*".
* "*" now matches any number of characters including zero.
* Case-normalize shell patterns before matching.
* Consistently case-normalize file names before matching.
* Adjust docs.
* Add tests.
