feat: upgrade MCP SDK to 1.29.0 for ChatGPT Streamable HTTP compatibility

Upgrade @modelcontextprotocol/sdk from 0.5.0 to 1.29.0, adding
StreamableHTTPServerTransport support so ChatGPT (protocol 2025-03-26)
can connect alongside Claude (SSE fallback). The SDK 1.x requires one
Server per transport, so SdkTransportRegistry now accepts a factory
function instead of a shared server instance.
