Add coveralls test coverage (from bedrock) and README badge

excoveralls as the coverage tool with a coveralls.json that skips the
generated leex/yecc .erl (their thousands of generated lines drowned
the signal: 43% with them, 77.4% without) and floors coverage at 77%.
The newest CI cell uploads via mix coveralls.github; only one cell
reports, since coveralls closes a PR build on the first non-parallel
upload. Also freshen dep requirements to the versions we test against
(iodata ~> 0.7, benchee ~> 1.5, ex_doc ~> 0.40) and update ex_doc.
