feat: add tree-shakeable package and docs imports (#313)

* perf(docs): lazy-load example code samples

Use docs-kit's split demo manifest so example pages keep a
small source index and defer highlighted code payloads to lazy
virtual demo modules.

* docs: refresh html filtering demo styling

* feat: add tree-shakeable package subpaths

Expose focused package entrypoints for the core SvelteMarkdown component
and each extension family, then verify optional peers only enter bundles
through the renderer paths that need them.

* docs: add tree shaking guide

* fix(docs): prevent duplicate sidebar active states

* fix: address tree shaking review feedback
