Commit Message

Refactor configuration

- Remove testrpc configuration file
- Make olympia rinkeby configuration inherit from local
- Make olympia related stuff optional for production
- Make email optional
- Make rpc ethereum node optional if there is already a configured IPC path
- Use eth addresses with checksum in settings and kubernetes settings
- Remove ULTIMATE_ORACLE_FACTORY
- Fix LMSR_MARKET_MAKER tests
- Remove not needed files (dokku)
