Major refactor

- Use builder pattern for `Attachment` and `Payload`.
- Use `Url` type where appropriate.
- Use idiomatic rust conversions where appropriate.
- Replace `rustc_serialize` with `serde` to eliminate extraneous fields from being sent to slack.
- Support three character hex codes
- Add a CHANGELOG
