Replace rustdoc in lib with readme

In order to enable maintaining crate level documentation in one place,
this commit moves some of the examples from lib.rs into the readme. It
then removes all of the rustdoc from lib.rs and instead pulls the entire
readme.md into lib.rs. This allows maintaining only the readme for both
the crate documentation and github documentation.
