test: add unit tests for subagent module

- 29 tests covering discoverAgents, resolveModel, runWithConcurrency, and createSubagentTool
- Export helper functions from subagent.ts for testability
- Tests cover: agent discovery from .md files with frontmatter, recursive directory scanning,
  model name mapping, concurrency limiting, unknown agent error handling, and tool schema
